Job Summary
The Dental Coordinator assists to connect Molina members with necessary dental services and coordinate as needed. They would help members who maybe need assistance scheduling or locating a dental provider. Case Management services are intended for members with significant medical, physical, dental, and/or behavioral diagnosis or diagnoses. Referrals for the dental program coordinator may be initiated by the member, member’s medical provider, dental provider, Case Manager, Call Center, or healthcare care professional and are based on a current, comprehensive evaluation and treatment plan. Care coordination services from the Dental Coordinator allow Medi-Cal members to call and gain access to extended dental services, assist members with locating a general or specialist dentist, accessing appointments, translation services, and transportation assistance.
Knowledge/Skills/Abilities
Conducts telephonic outreach to members who may need dental coordination assistance
Verify eligibility, enrollment history, demographic data, primary care physician, and current health status of members
Assess members needs for possible handoff to ongoing Case Management
Makes recommendations &/or referrals based on assessment/screening findings
Assist in education of available benefits and dental providers
Will assist members in locating appropriate providers and/or making appointments, as applicable
Accurately completes and maintain documentation related to member interaction, assessment/screener and provider contacts
Creates and completes timely follow-up with members
Establishes relationships with members, providers, and interdisciplinary care team members
Responsible to review reports to identify members that may need dental coordination assistance
Collaborate with other individuals within HCS and other departments
Provide general administrative support for the Health Services team
Perform any other job-related tasks, as requested
Job Qualifications
Education and Experience Requirements :
High school diploma or general equivalency degree (GED) is required
Associates Degree or greater is preferred
A minimum of two (2) years of healthcare experience is required
Previous dental, case management, pediatric, and/or Medicaid managed care experience is preferred
Required Competencies / Knowledge / Skills :

Typing a minimum of 35wpm proficiently
Ability to talk and data enter simultaneously
Intermediate proficiency level with Microsoft Office products, including Word and Excel
Knowledge of medical terminology is required
Ability to gather work related information and coordinate workflow and assist Health Services team

PHYSICAL DEMANDS:
Working environment is generally favorable and lighting and temperature are adequate. Work is generally performed in an office environment in which there is only minimal exposure to unpleasant and/or hazardous working conditions. Must have the ability to sit for long periods. Pay Range: $21.16 - $38.37 / HOURLY
