Job Description

Basic Requirements

  1. Citizen of the United States

  2. Must be proficient in written and spoken English.

  3. Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).

Preferred Experience:

  1. Minimum 2 years of recent, successful clinical nursing practice as an LVN/LPN

  2. Demonstrated ability to be self-directed and require minimal supervision

  3. Demonstrated experience in providing patient education and health coaching

  4. Demonstrated ability to work well with patients/families and with other health care professionals

  5. Documented evidence of providing consistent excellent customer service

Work Schedule: Full Time, Day shift, 7:30 am - 4:00 pm, may include weekends and holidays.

Duties: The LVN will function as a member of the Primary and Ambulatory Care Team. He/she ensures that Veteran-centered health care is provided and delivered in a quality, timely, accessible, and respectful manner, consistent with the mission, goals, and objectives of the Veteran's Health Administration. He/she is responsible to the Clinic Nurse Manager.

  1. Delivers a broad range of vocational nursing care to a group of patients

  2. Monitors patients for illness or incapacity

  3. Evaluates and observes patients, identifying obvious and subtle deviations

  4. Recognizes emergent situations and initiates appropriate interventions

  5. Uses practical knowledge of human motivation behavior and reactions to situations

  6. Acts as a patient advocate in facilitating patient care

  7. Exercises self-direction in performing the most complex LVN assignments involving both standard and non-standard specialized activities

  8. Participates in identifying, developing, and conducting self-reliance health related teaching programs for patients and families

  9. Contributes to the interdisciplinary team in developing, implementing, and evaluating patient treatment plans

  10. Maintains and orders adequate supplies, drugs, and equipment necessary for clinic functions

  11. Participates in clinic Quality Assurance activities

  12. Assists in maintaining accurate clinic logs and statistics as required

  13. Reviews patient records for preventive care, tracts preventive care and conducts patient education associated with chronic disease

  14. Performs other duties as assigned depending on nursing care demands and/or needs

  15. Performs EKGs, medication administration and implementation of orders for patient care

  16. Prepares rooms and patients for examinations and takes responsibility for coordinating smooth patient flow in the clinic area
