Senior Backend Engineer

Position Overview

We seek a skilled Backend Engineer to build and scale the infrastructure powering our AI healthcare platform that serves millions of patients. This role requires expertise in Python, API development, and distributed systems design with a focus on healthcare data processing and security. You’ll work at the intersection of traditional backend engineering and AI systems, building the scalable foundation that enables our AI doctor platform to handle 14+ million patient interactions while maintaining the highest standards of security and reliability.

What You’ll Build

We’re scaling rapidly and tackling complex infrastructure challenges. You’ll work on:

Healthcare API Infrastructure

  • Scalable healthcare APIs using Python, FastAPI, and AWS, with the ability to handle millions of daily requests

  • RESTful API development and expansion to support our growing suite of AI healthcare tools

  • Real-time data processing systems managing sensitive patient information with HIPAA compliance

  • Internal API architecture supporting our web platform and AI coordination systems

Integration & Data Systems

  • Secure health record exchange integrations with EHR systems, QHINs, and healthcare networks

  • Third-party platform integrations, including telehealth providers, analytics platforms, and healthcare APIs

  • Large-scale sensitive data processing pipelines with encryption and compliance safeguards

  • Partner API integrations across dozens of well-documented but diverse healthcare systems

Scale & Reliability Architecture

  • Database architecture optimization for high-volume healthcare data with complex query patterns

  • Redis implementation for secure caching and session management in healthcare environments

  • Robust error handling, failover systems, and disaster recovery for mission-critical healthcare services

  • Performance optimization and scaling strategies for a rapidly growing user base

AI System Integration

  • Backend coordination systems that guide and manage AI tools and decision-making workflows

  • Python-based deterministic systems that work alongside AI agents for clinical accuracy

  • API development supporting AI model inference, data preprocessing, and result processing

  • Infrastructure for AI safety systems including validation layers and human escalation protocols

These examples represent our current challenges. You’ll also work on healthcare compliance automation, advanced monitoring systems, multi-region deployment strategies, and building the backend foundation for next-generation AI healthcare features.

Required Qualifications

Education & Background

  • Computer Science degree OR exceptional work experience demonstrating equivalent technical depth

    • Engineering, Mathematics, or related technical degrees are also welcomed with a strong programming foundation

  • Strong preference for candidates who have started their own company, been a founding engineer, or worked extensively in startup environments

  • Demonstrated ability to work independently, solve complex problems without hand-holding, while also collaborating effectively with cross-functional teams

Technical Expertise

  • 5+ years of backend engineering experience with expertise in Python and API development

  • Strong experience with FastAPI, Flask, or similar Python web frameworks

  • Proven experience with cloud services

  • Experience building and scaling RESTful APIs, handling high-volume traffic

  • Proficiency with relational databases (PostgreSQL, MySQL) and optimization techniques

  • Knowledge of Redis, caching strategies, and distributed systems design

System Design & Integration

  • Experience with third-party API integrations and webhook management

  • Understanding of microservices architecture and distributed system patterns

  • Knowledge of security best practices for sensitive data handling and API design

  • Experience with CI/CD pipelines, containerization (Docker), and deployment automation

  • Familiarity with monitoring, logging, and observability tools

Preferred Qualifications

  • Previous experience building systems for healthcare, fintech, or other regulated industries

  • Knowledge of healthcare data standards (HL7, FHIR) and HIPAA compliance requirements

  • Experience with high-scale data processing and real-time systems

  • Background working with AI/ML systems and model serving infrastructure

  • Experience with message queues, event-driven architectures, and async processing

  • Knowledge of healthcare workflows, EHR systems, or telehealth platforms

  • Experience with seed/Series A stage companies in healthcare or technology

  • Advanced degree in Computer Science, Engineering, or related technical field
