This is a full-time 10-hr Day shift position, working 40-hrs per week, 1 day remote. Located at Scripps Green Hospital in La Jolla supporting the Organ Transplant Clinic department.

What will you do:
Formulate custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care.
Establish specific, measurable, attainable, relevant and time specific nursing goals to guide the care planning process and promote self-reliance.
Assist patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance.
Identify and align resources that impact high-risk care.
Coordinate services on behalf of the high-risk patients.
Work in collaboration with the Healthcare team to track and review data for research projects as well as resource utilization related to relevant specialty and make recommendations as needed.
R*equired Education/Experience/Specialized Skills:*
- BSN
- CA RN License
- At least three years recent clinical nursing experience in a variety of clinical settings
Required Certification/Registration:
- BLS through AHA
- Two years experience managing care of relevant transplant patient population
